Spanish and multi-lingual calls

Add Spanish (or another language) in Settings → AI Voice → Languages. When you list more than one language, the AI auto-detects what the caller is speaking on the first turn and matches their language for the rest of the call. The bilingual voice (Luis) handles English and Spanish equally well.

Updated May 20, 20263 min read

The AI can hold a complete conversation in Spanish — same number, same configuration, no manual switching. About 1 in 5 FrontDesk practices have multi-lingual enabled.

Enable Spanish (or another language)

  1. Settings → AI Voice → Languages
  2. Click + Add language → pick the language.
  3. Choose a bilingual voice that supports both. Luis is recommended for English + Spanish.
  4. (Optional) Set a primary — used as the default for the first greeting before detection.
  5. Save.

The change is live on the next inbound call.

How language detection works

MomentWhat happens
Phone ringsAI plays the greeting in the primary language
Caller speaks first sentenceSpeech-to-text identifies the language
AI generates responseSwitches voice + language to match the caller
Rest of the callStays in the detected language unless the caller switches mid-call

Detection takes ~0.5 seconds and is correct >97% of the time on a clear first sentence. If the caller mumbles a single word ("hello"), detection may pick the primary by default — that's fine, the caller will trigger a switch on the next turn.

Greeting that works for both languages

Two approaches:

Bilingual greeting (recommended for ~50/50 patient populations):

Hello and welcome to {{practiceName}}. Hola, gracias por llamar a {{practiceName}}. How can I help you today?

The AI starts in the primary language; the caller's response switches the rest of the call.

Single-language greeting (for ~90/10 populations):

Hello and welcome to {{practiceName}}, how can I help you today?

Patients who respond in Spanish trigger a clean switch on turn 2. Slightly less explicit but more natural.

Train your FAQ in both languages

Settings → AI Voice → Train → [FAQ] → Add translation

For each FAQ, you can add a Spanish version. The AI uses the matching version based on call language. If only English exists, the AI translates on the fly — usable but less polished.

Highest-value translations:

  • Hours and address
  • Accepted insurance plans
  • New-patient paperwork instructions
  • Cancellation / no-show policy
  • Parking and arrival instructions

Reminders in the patient's language

Settings → Messaging → Reminders → [template] → Add translation

Patients are tagged with their preferred language based on the language of their first call. Future reminders use that language automatically. You can override the tag on the patient profile.

Online booking in Spanish

The patient-facing booking page detects browser language and shows Spanish or English. Form fields, service names, and confirmation messages all translate. To customize Spanish copy beyond the defaults, Settings → Online Booking → Translations.

Additional languages

Available on Professional+ with regional voice support:

LanguageVoice options
FrenchStandard, Canadian-French
PortugueseBrazilian, European
MandarinMainland, Taiwanese
VietnameseNorthern, Southern
TagalogStandard

To enable, Settings → AI Voice → Languages → + Add language. If you don't see one you need, contact support.

Tips

  • Always do a test call in each language after changing settings. Run a script from Settings → AI Voice → Call Scripts in the target language.
  • Listen for proper noun pronunciation. Provider names, street names, and procedure names sometimes pronounce oddly in the non-primary language. Add pronunciations in Settings → AI Voice → Pronunciation.
  • Watch acceptance rates. If Spanish-speaking patients drop off, check that the Spanish greeting feels welcoming and the booking flow works. Tweak templates and re-test.

What's next

Frequently asked questions

Do I need a separate phone number for Spanish callers?
No — the same number works for both. The AI detects the caller's language automatically. Some practices still buy a dedicated Spanish number for marketing reasons, but it's not technically required.
Which languages are supported?
English and Spanish are fully supported with bilingual voices. Additional languages (French, Portuguese, Mandarin, Vietnamese, Tagalog) are available with regional voice options on Professional+ plans.
Will my custom FAQs work in Spanish?
They'll work, but accuracy is best when you add a Spanish version of each FAQ. Settings → AI Voice → Train → click any FAQ → Add translation.
