Letter of Authorization
Authorization to port your business phone number to the Front Desk voice service.

What Is a Letter of Authorization?
A Letter of Authorization ("LOA") is a document that authorizes Front Desk and its telecommunications carriers to act on your behalf to transfer ("port") one or more telephone numbers from your current phone service provider to the Front Desk Voice Service. Carriers require a signed LOA before they will release a number from your existing provider.
Authorization Statement
By signing a Front Desk Letter of Authorization, I verify that I am, or am authorized to represent, the named customer, and that I am authorized to change the primary carrier for the telephone number(s) listed on the LOA. The name and address I provide is the name and address on record with my current carrier for each telephone number listed.
I authorize Front Desk and its designated telecommunications carriers to act as my agent on my behalf to: (a) notify my current carrier of my decision to change my preferred carrier for the listed number(s); and (b) obtain any information reasonably necessary to complete the carrier change, including customer name and number, billing records, service records, and network and equipment records.
I understand that only one carrier may be designated as the preferred carrier for any one telephone number. I understand that my current carrier may charge a fee in connection with the change of preferred carrier, and that I may consult my current carrier as to whether any such fee applies. This LOA becomes effective on the date of signature and remains in effect unless revoked in writing prior to completion of the port.
What You'll Need to Port a Number
- The telephone number(s) you want to port;
- The business name and service address exactly as they appear on record with your current carrier;
- Your current carrier account number and, if applicable, your port-out PIN or passcode;
- A recent copy of your phone bill (some carriers require this);
- A signed Letter of Authorization.
Porting timelines vary by carrier and number type, and your number remains active with your current provider until the port completes. Do not cancel your existing phone service until Front Desk confirms your port is complete — cancelling early can cause the number to be released and become unavailable for porting.
