The session writes the first draft; you review and sign. Type a two-minute recap, dictate one, upload a recording, or let note-taking listen during the visit, and the note editor fills in your template — DAP, SOAP, BIRP, GIRP, intake, or your own — with what was said and what the chart already knows. Nothing is stored but the note you sign.
AI progress notes are included on Pro and above. The practice turns them on under Settings → Documentation.
Turn it on
Settings → Documentation → AI progress notes has two switches:
| Switch | What it does |
|---|---|
| AI progress notes | Enables the Draft with AI card in the note editor. Off by default. |
| Live note-taking during visits | Allows the Live tab and the AI notes item in a video visit. On by default once AI progress notes is on; turn it off to allow only recaps, dictation, and uploads. |
If the switches are disabled with a plan notice, your plan does not include the feature yet; see Upgrade or downgrade your plan.
Four ways in
Open a clinical note as usual (see Write and sign clinical notes). While you are editing, a collapsible Draft with AI card sits above the sections with four tabs:
| Tab | What you do | Metered |
|---|---|---|
| Recap | Type a short summary of the session and click Draft note. | No |
| Dictate | Press the mic, speak a summary, stop. The transcript appears for you to correct, then Draft note. | Yes |
| Upload | Pick an audio file (webm, ogg, wav, mp3, m4a, or mp4, up to 25 MB). It is transcribed, shown for correction, then Draft note. | Yes |
| Live | Tick the consent attestation and click Start listening. A pulsing "Listening · audio is not saved" indicator runs while the transcript builds; Stop, then Draft note. | Yes |
The draft uses the template already chosen in the editor, so pick the template first. Alongside your recap or transcript, the draft is grounded in the chart: the client's diagnoses, the active treatment plan's goals and objectives, the latest outcome measure scores, the previous signed note, and the visit's date, length, and modality.
If a section already has text, Draft note asks whether to Replace it or Append to it. A section the recap or transcript did not cover is left as [Clinician to complete] rather than invented. Once applied, the editor shows AI draft · review every section before signing, and any warnings in amber.
Live note-taking in a video visit
In a one-to-one video visit, the More menu has an AI notes item. It opens the consent attestation, then starts listening to your microphone for the visit's client and appointment. While it is on:
- a chip near your timer reads AI notes · audio not saved;
- the client's screen shows AI note-taking is on · audio is not saved for the whole time, and the chip disappears when you stop or the visit ends.
Ending the visit stops listening. Click Start clinical note on the ended screen and the editor's Live tab shows Transcript from your video visit (N min) with Draft note ready. The handed-over transcript lives only in your browser until the draft is made, and signing out clears it.
The item is hidden for group visits and when the practice's live note-taking switch is off.
Consent
Live note-taking will not start until you tick: "I told the client that AI note-taking is on today and they agreed. Audio is not saved." The attestation is written to the audit trail and stamped on the client's record, where it shows as verbal consent with the date and who attested.
For written consent, the seeded Consent for AI-Assisted Notes intake form can be sent like any other form. A signed form records consent on the client's record as well.
The telehealth consent line and the Consent for Telehealth form also say that the practice does not store audio or video of visits and that the client will be told when AI note-taking is on.
What is and is not stored
- Audio: never. Dictation, uploads, and live audio stream to the transcription service and are dropped; nothing is written to disk on any path.
- Transcript: discarded. It exists in your browser until the draft is made, and on the server only for the moment of a request. Nothing persists it.
- The note: yes. Only the note you sign is kept, marked AI-assisted in the note list and read view, with your name and the time stamped as the reviewer when you sign.
- The audit trail records that AI processing happened, the mode, and the minutes — never what was said.
Transcription and drafting run on a vendor covered by FrontDesk's Business Associate Agreement.
Review and sign
A draft is a starting point, never the record. Read every section, correct the clinical judgment, and sign as usual. When the note's source is AI, the sign dialog carries the line "I have reviewed this AI-assisted draft and it reflects my clinical judgment."; signing is the attestation. Everything else about signing — locking, addenda, voiding, co-signature, charge capture — works as described in Write and sign clinical notes.
Minutes and plans
Typed recaps are not metered. Dictation, uploads, and live sessions use AI note minutes, shown under Settings → Billing beside your other allowances; see Understand usage and overages.
| Plan | Included each month | Extra minutes |
|---|---|---|
| Trial | 60 | — |
| Pro | 300 | 15¢ per minute |
| Elite | 1,000 | 10¢ per minute |
| Enterprise | Custom | Custom |
The pool is soft: a live session is never cut off. Overage is invoiced with your other usage.
What it will not do
- Psychotherapy notes never get AI drafting; the card does not appear in a psychotherapy note.
- Group sessions are not supported for live note-taking. Starting it on a group appointment is refused with "Group sessions are not supported". Type a recap after the group instead.
- Soften risk. If the recap or transcript mentions self-harm, suicidal ideation, abuse, or a threat to others, the draft keeps it and adds the warning "Possible risk content in the session. Confirm the risk assessment and safety plan before signing."
Troubleshooting
| What you see | What to do |
|---|---|
| Dictate or Live cannot use the microphone | Allow the microphone for the app in your browser's address bar, then try again. Recap and Upload need no microphone. |
| The card shows an upgrade notice | Your plan does not include AI notes. It is on Pro and above; see Upgrade or downgrade your plan. |
| The card points you to Settings → Documentation | The practice switch is off. An owner or admin turns on AI progress notes there. |
| No Live tab, or no AI notes item in the visit | The linked visit is a group, or Live note-taking during visits is off in Settings → Documentation. |
| "Group sessions are not supported" | Live note-taking is one-to-one only. Draft from a recap after the group. |
| "Could not start" or the session stops on its own | The listening ticket lasts ten minutes from when it is issued and one live session runs per browser tab. Stop and start again; if it keeps happening, check the connection and see Video visit problems. |
| A section reads [Clinician to complete] | The recap or transcript did not cover it. Fill it in yourself, or add detail to the recap and draft again with Append. |
| The note shows AI-assisted but I wrote most of it | The tag reflects that any part was drafted by AI. Your edits and signature are what make it the record. |
