Manage your template library

Settings → Documentation → Template Library is where every document template lives, grouped as scored measures, intake forms, progress notes, consent forms and other documents. Browse pre-built adds templates from the library; Build new template opens the form builder with short and long text, email, phone, date, dropdown, multi-select, radio, checkbox, signature, file upload, heading and paragraph fields. Each row shows fields, minutes, version, how many were sent and the completion rate, and its menu offers Send to patient, Preview, Public link, Edit, Duplicate, Version history and Delete. A new version is only created when you change a form's structure after it has been completed at least once, so earlier submissions keep the exact form they answered; older versions can be restored. Deleting removes the template from the library but keeps completed records.

Every document your practice sends or fills in starts as a template. Open Settings → Documentation → Template Library (also reachable from the Settings menu under Client Care).

What is in the library

Templates are grouped into Scored measures, Intake forms, Progress notes, Consent forms and Other documents; the pills at the top filter the list and show counts, and the search box finds a template by name. Each row shows the name, a DEFAULT tag when the template is in your default intake package, whether it has a public link or a signature field, and the numbers: fields, estimated minutes, version, how many were sent, the completion rate, and when it was last edited. Clicking the version opens its history; clicking the sent count opens Shareable Documents filtered to that template.

Add a template

  • Browse pre-built adds ready-made templates (standard intake, consents, and scored measures such as symptom questionnaires) to your library.
  • Build new template opens the form builder. Drag fields from the palette: Short Text, Long Text, Email, Phone, Date, Dropdown, Multi-Select, Radio Buttons, Checkbox, Signature, File Upload, and layout elements (Heading, Paragraph). Click Save Template when done.

Hover a row for the quick actions, or open its ⋮ menu: Preview, Send to patient, Public link, Edit, Duplicate, Version history, Delete.

Edit, versions and restore

Editing a template that has never been completed simply changes it. Once a template has at least one completed submission, an edit that changes its structure creates a new version: the old one is kept, and every past submission stays attached to the exact form it answered. Version history lists each version with its field and submission counts and a Restore button on older ones; restoring creates a new current version from that snapshot.

Duplicate and delete

Duplicate creates "name (Copy)" with statistics reset, outside the default package and without triggers, for a variant you want to keep alongside the original. Delete asks you to confirm and then removes the template from the library; completed records that used it are preserved.

For a form that anyone should be able to fill in without a portal login, open Public link on the template, switch it on, and copy the address (it lives on your booking site). The modal shows how many submissions arrived through the link, and Regenerate link replaces it, which stops the old address working.

Sending

Send to patient on a row sends that one form by SMS, email or a copied link; the link expires after seven days and a resend issues a fresh one. To send several documents in one message and track them together, use Send documents on the client's profile: see Send documents to a client. The default intake package, and the switch that lets the AI send it automatically, live under Shareable Documents on the same Documentation menu.

Clinical note settings

The card at the bottom of the page holds two timers for clinical notes: how many hours after a visit to nudge the provider about an unsigned note, and every how many days to remind supervisors about co-signatures. See Notes due and co-signatures.

Frequently asked questions

When does editing a template create a new version?
Only when the edit changes the form's structure (fields added, removed or changed) and the template has already been completed at least once. The old version is snapshotted and every past submission keeps the exact form it was answered on. Wording-only edits and edits to a never-used template simply update it.
Can I go back to an earlier version?
Yes. Click the version badge on the row to open Version history, then Restore on an older one. That creates a new current version from it; past submissions are untouched.
What happens to completed forms when I delete a template?
They are preserved. Delete removes the template from your library so it cannot be sent again; completed records that used it stay on the clients' profiles and in Shareable Documents.
Can someone fill out a form without a portal login?
Yes, with a public link. Open the template's menu, choose Public link, and switch on "Anyone with the link can fill out this form". Copy the link and put it on your website or in an email; the modal counts submissions that came through it. Regenerate link makes a new address and stops the old one working.
What is the difference between Duplicate and a new version?
Duplicate makes a separate template named "(Copy)" with its own statistics starting at zero, outside the default package and without any triggers. A version is the same template's history. Duplicate when you want a variant to send alongside the original.
How do I send one form to one client from here?
Use Send to patient on the row. It offers SMS, Email or Copy Link; the link expires after 7 days (a resend issues a fresh one). For a set of documents in one message, use Send documents on the client's profile instead.
What are the hours and days settings at the bottom of the page?
They belong to clinical notes: how many hours after a visit to nudge a provider about an unsigned note, and how often to remind supervisors about notes waiting for a co-signature. Change them and click Save.
